Herniation of the lumbar intervertebral disc compressing a lumbar or sacral nerve root, causing radicular leg pain (sciatica). L4/5 and L5/S1 account for 95% of lumbar disc herniations. Peak age 30-50. 90% resolve with conservative management within 6-12 weeks. True sciatica = leg pain > back pain.
