Fat Pad Impingement (Hoffa's)
Pinched Fat Pad Below Kneecap
Irritation and impingement of the infrapatellar fat pad (Hoffa's fat pad) between the femoral condyle and tibial plateau. The fat pad is the most pain-sensitive structure in the knee (richly innervated). Pain is ANTERIOR and INFERIOR to the patella, worse with EXTENSION (opposite of PFPS which is worse with flexion). Often overlaps with patellar tendinopathy.
